How do I get a restraining order against a non family member that is haring and stalking me?

I once was friends with this person and when I decided to completely cut her off she began riding ped my house, calling my job and my parents spreading horrible rumors about me. She has threatened to do bodily harm to me and is very persistent about trying to ruin all that I have worked for. Im not afraid of her but I am afraid of what she capable of doing to my reputation and my home life. She is a serious crazy person.
