I think i have type one diabetes!! please help urgent?

hi i am a 13 year old female and i think i have diabetes. i have been really moody lately and have had alot of headaches.also i have gotten really dizzy like so bad i see spots and black out. i have lost 3 pounds in one week.also yesterday i had 2 Hershey kisses and took a nap for 3 hours.(that is not normal)please help me! i am going in to the doctor for a physical tomorrow what should i say?what tests should i ask for?
